Federal law requires this consent form be provided to you. Unless authorized by law, we cannot use your tax return information for purposes other than the preparation and filing of your tax return without your consent. You are not required to complete this form to engage in our tax return preparation services.

If we obtain your signature on this form by conditioning our tax preparation services on your consent, your consent will not be valid. Your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature. If you do not consent, you may still have your tax return prepared and electronically filed by us for a fee.

For your convenience, we have entered into an arrangement with third parties to provide qualifying taxpayers with the opportunity to apply for an Electronic Refund Disbursement Service and/or Loan product. To determine whether these products may be available to you, we will need to use your tax return information in order to calculate the amount of your anticipated refund. If you would like us to use your tax return information to determine whether these products may be available to you while we are preparing your return, please sign and date this consent to the use of your tax return information.

By signing below, you authorize us to use the information you provide to us during the preparation of your 2025 tax return to determine whether to present you with the opportunity to apply for these products and services.

If you believe your tax return information has been disclosed or used improperly in a manner unauthorized by law or without your permission, you may contact the Treasury Inspector General for Tax Administration (TIGTA) by telephone at 1-800-366-4484, or by e-mail at complaints@tigta.treas.gov.

If you consent to the disclosure of your tax return information, Federal law may not protect your tax return information from further use or distribution.

You are not required to complete this form to engage in our tax preparation services. If you agree to the disclosure of your tax return information, your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature.

You have indicated that you are interested in receiving an Electronic Disbursement Service and/or Loan (or collectively, “Products or Services”) from Pathward, National Association. In order to provide you with the opportunity to apply for one of these Products or Services, we must disclose all of your 2025 tax return information necessary for evaluating the request to Pathward.

If you request a more limited disclosure of tax return information, you will not be eligible to submit an application request for these Products or Services. If you would like us to disclose your 2023 tax return information for this purpose, please sign and date your consent to the disclosure of your tax return information.

By signing below, you authorize us to disclose to Pathward all of your 2025 tax return information necessary for the evaluation and processing of your request for a Product or Service. You understand that if you are unwilling to authorize the disclosure and sharing of your tax return information with Pathward, you will not be able to obtain a Product or Service, but you may still choose to have your tax return prepared and filed by us for a fee.

Section 1: Scope of Service

Lawson Mobile Tax (LMT) will provide comprehensive tax preparation services for the tax year, including reviewing your financial documents, preparing your federal and state tax returns, and filing them electronically (or by mail, if necessary). Our services cover:

  • Income tax return preparation (Federal and applicable State returns)

  • Review of deductions, credits, and exemptions based on your provided information

  • Communication with tax authorities as needed (if authorized by client)

Section 2: Fee Structure and Payment

Fee Structure: Our fees are based on the complexity of your tax return, with options to suit your convenience. The fee structure is outlined below:

  • Standard Tax Preparation Fee: Starting at $480 for basic individual tax returns

  • Complex Return Fee: Starting at $699 for returns that include business income, rental income, or other complex cases

  • Additional Services: Fees for extra services, such as tax consulting or audit support, will be communicated prior to service.

Payment Terms: For clients expecting a refund, payment is automatically deducted from your refund at the time of disbursement from the IRS. If no refund is due, fees are collected at the time services are rendered. Please note that, as all services are final, no refunds will be issued after services have been completed

Section 3: Client Responsibilities

Client Responsibilities: As our client, you play a critical role in ensuring accurate tax preparation. You agree to:

  • Provide all required documentation and information in a timely manner

  • Disclose any income, deductions, credits, and other relevant financial information fully and accurately

  • Review the completed tax return before filing and confirm that all information is accurate

Failure to provide accurate or complete information may result in additional fees and penalties from tax authorities. LMT is not responsible for any inaccuracies resulting from incomplete or incorrect information provided by the client.

Section 4: LMT Responsibilities

LMT Responsibilities: LMT is committed to handling your tax preparation with the utmost professionalism, confidentiality, and accuracy. Our responsibilities include:

  • Providing accurate and timely tax preparation based on the information you provide

  • Ensuring compliance with all applicable tax laws and regulations

  • Maintaining strict confidentiality regarding your financial information, in compliance with data protection standards

Section 5: Confidentiality and Data Security

Confidentiality and Data Security: LMT is dedicated to protecting your personal and financial information. We use secure technology to safeguard your data and will not share it with unauthorized third parties without your written consent.


We retain your information as required by law, and you may request deletion of your data after tax processing is complete, in accordance with our data retention policy.

Section 6: Termination Clause

Termination Clause: Either party may terminate this agreement at any time with written notice. If terminated by the client, LMT reserves the right to invoice for any work completed up to the date of termination.


If LMT terminates the agreement due to non-compliance or failure to provide required documents, the client may be liable for additional fees if work has commenced.
